Chronic diseases are defined as conditions which last 1 year or more and require ongoing medical attention or limit activities of daily living. There are a number of chronic diseases which we monitor at the practice such as Diabetes, Heart Disease, Hypertension, and Asthma. See Below for the full list.
An annual review with us usually involves:
- an assessment of how you are managing the condition over the last year
- some standard tests like blood pressure, weight or blood test
- and a review of the medication you maybe taking
- Reviews are overseen by our experienced Nurses, Clinical Pharmacist and GP's depending on the condition you have
In order to improve the efficiency of the process and help us manage the increasing demand on our services, we have the devised several online forms below which you can enter information before your appointment or as an alternative to meeting us if you feel a face to face meeting is unecessary.
Rest assured, if you meet the criteria, each year you will be contacted by the surgery for an annual review, via a letter from us or a text usually on the month of your birthday.
