Jury Service Medical Exemption Request Form

Important Information:

In Scotland, patients can apply to be excused from jury service if ill health or disability would prevent them from serving. The application is made to the court, not to the GP. The Scottish Courts and Tribunals Service states that requests on health grounds should be supported by appropriate medical evidence. Completion of the form below is required but does not guarantee that a medical certificate or supporting letter will be provided.

A GP can only support an application where there is sufficient medical evidence that jury service would not be possible or would pose a significant risk to the patient's health or wellbeing.

In some cases:

  • We may decline to provide a letter.
  • We may ask for additional information.
  • We may require a consultation before making a decision.

Can I be excused from jury service?

The court may consider excusal where ill health or disability would make it difficult or impossible for you to carry out jury service. Applications are considered individually and you may be asked to provide supporting evidence

Do I need a letter from my GP?

Not always.

The court will decide what evidence is required. In some cases, the information you provide is often sufficient. Where medical evidence is requested, the court may ask for a letter or certificate from your doctor or another healthcare professional. 

How do I apply for excusal?

  1. Read the information enclosed with your jury citation.
  2. Contact the court using the details provided on the citation.
  3. Explain why you are requesting excusal.
  4. Provide any supporting evidence requested by the court  paper copy to our reception or via word/PDF/photo  sent to our email on loth.clinicals70272@nhs.scot
  5. If the Courts have requested a letter from your GP, please complete the form below.

Do I need to book an appointment?

 
Please you do not book a GP appointment solely to request a jury service exemption letter. Where medical evidence is required, please submit on the online form request to the practice and include a copy of your jury citation if possible.
