Jury Exclusion letter request

Jury Exclusion letter request

Completion of this form helps us to consider your case but does not automatically guarantee that a medical certificate or supporting letter will be provided.

A GP can only support an application where there is sufficient medical evidence that jury service would not be possible or would pose a significant risk to the patient's health or wellbeing.

We require 4-5 working days to consider your case , review your records and produce a letter.

In some cases:

  • We may decline to provide a letter.
  • We may ask for additional information.
  • We may require a consultation before making a decision.
