The Doctors at Bangholm have access to your medical records and therefore are the first point of contact for any changes to your medications and treatments. They can help with anticipatory care planning, referring to services and keeping up with your general health whilst you receive palliative care. The doctors are able to speak to you via telephone, video call, in the practice or in your home, depending on your needs.
The district nurses are dedicated to patients who have difficulty leaving the house. They visit you in your own home and can help with wound dressing, administering and identifying issues with medications, looking after medical devices such as catheters and syringe drivers. They can also help obtain equipment such as commodes and hospital beds to help you stay at home safely for longer. They may also assist with assessments for care packages and refer to physiotherapists and occupational therapists.
District nurses are there for you and your family, helping you to understand what to expect and guiding you through it. District nurses can visit from once a week to multiple times a day depending on your needs.
