What is Anticipatory Care Planning (ACP)?

Anticipatory care planning is where you have discussions surrounding your future care. These discussions can take place with family and loved ones as well as healthcare professionals such as your doctor, nurse or care worker. The team responsible for your care would like to know what is important to you when they make plans regarding your future care and treatment.

Making plans in advance means that there is less to think about and consider if you become ill. Everyone has their plans and has different ideas about what is important to them.

What Do ACPs Include and How Can I Start The Process?

ACPs may include the following:
● Personal information about what matters to you and your family
● Where and how you can be cared for
● A carer information and support plan
● An emergency treatment and care plan
● Decisions about CPR
Your regular GP, nurse or care worker might suggest setting up a meeting to discuss ACP. You are welcome to bring a family member or loved ones to the consultation if you wish.
